BAYADA Home Health Care is seeking a Medical Social Worker for a per diem opportunity performing home health visits. This role involves working with clients in designated geographic territories, including Forsyth, Stokes, Surry, Wilkes, and Yadkin counties.

As a Medical Social Worker, you will be part of a multi-disciplinary healthcare team providing skilled nursing and rehabilitative care to clients. Your responsibilities will include:

  • Making home visits to clients in designated territories
  • Assisting the team in understanding social and emotional factors related to clients' health problems
  • Providing clinical social evaluations and planning interventions based on evaluation findings
  • Identifying community resource referrals to address clients' and families' practical and environmental needs
  • Acting as an advocate for clients and families to navigate the patient through the community system
  • Participating in the development of the total plan of care with the multidisciplinary team
  • Developing and maintaining contracts with public and private agencies as resources for patients
  • Educating and instructing patients, family members, or other patient representatives in community resources to promote patients' health, safety, well-being, and independent living
  • Accurately documenting observations, interventions, and evaluations pertaining to patient care management and services provided

Qualifications for this role include:

  • A minimum of a Master's in Social Work, with at least 2 years of community-based experience
  • Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree
  • Home care experience a plus
  • Demonstrated ability to read, write, and effectively communicate in English
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred
  • Ability to travel to cases, as assigned
