About the Position
We are seeking a highly skilled and experienced Dialysis Care Professional to join our team at the Veterans Affairs, Veterans Health Administration. As a key member of our interdisciplinary team, you will provide exceptional care to patients undergoing dialysis treatment and develop individualized care plans to meet their unique needs.
Key Responsibilities
- Deliver direct patient care to patients undergoing dialysis treatment, including assessing their physical and emotional needs, developing care plans, and implementing treatments.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and delivery, including attending team meetings, communicating with healthcare providers, and participating in quality improvement initiatives.
- Maintain accurate and up-to-date patient records, including medical history, medication lists, and treatment plans, and ensure compliance with regulatory requirements.
- Participate in education and training programs to enhance your skills and knowledge, including workshops, conferences, and online courses.
- Contribute to the development and implementation of policies and procedures to improve patient care and safety.
