About the RoleAs a Care Coordination Specialist at Philadelphia Corporation for Aging, you will play a vital role in assessing the needs of individuals aged 60 and over who seek Home and Community Based Services. Your primary responsibility will be to conduct assessments to determine the level of care required and make appropriate referrals to ensure seamless transitions.

Your expertise will be instrumental in evaluating the availability and appropriateness of Home and Community Based Services or facility placement to best meet applicants' needs. You will also complete financial screening tools for applicants interested in Personal Care Home, Domiciliary Care placement, or Home and Community Based Services.

The successful candidate will have excellent communication skills, both written and verbal, and the ability to organize multiple client-related activities. A bachelor's degree in behavioral science, human services, social work, or related fields is essential. Additionally, a minimum one year of social work, case management, or assessment/intake experience is required.

Salary and Benefits$41,192.55- $47,685.53 per annum depending on skills and experience. The package includes medical, prescription drug, vision, and dental coverage, flexible spending plan (health and dependent care), EAP, life insurance, short- and long-term disability insurance, pre-tax commuting and parking benefits, and a 403(b) retirement plan with employer contributions.
