Hospice Administrator Job Description

At CommuniCare Health Services, we are seeking a highly skilled and compassionate Hospice Administrator to lead our hospice care team in Cleveland, OH.

Job Summary:

The Hospice Administrator is responsible for overseeing the implementation, coordination, and supervision of all aspects of patient and family services in accordance with hospice standards. This role requires strong leadership and administrative skills, as well as the ability to manage a team of healthcare professionals.

Key Responsibilities:

  • Supervise the planning, coordination, and delivery of patient and family care.
  • Interview, assess, and evaluate potential patients and families, or delegate responsibility to appropriate staff.
  • Employ qualified personnel and provide for adequate staff education, ensuring the adequacy of all staff.
  • Assign each patient and family to an appropriate staff member, or delegate responsibility to appropriate staff.
  • Conduct regular interdisciplinary team meetings for the development, review, and revision of patient and family plans of care.
  • Implement and monitor compliance with agency policy and procedures.
  • Conduct staff meetings for information and staff support as needed.
  • Conduct visits to patients and families as necessary, per patient and family needs, staff needs, or for staff supervision.
  • Supervise the maintenance of medical records, ensuring accuracy, completeness, and compliance with agency policy and/or licensing regulations.
  • Provide verbal and written performance evaluations for assigned interdisciplinary team members.
  • Promote the utilization of resources for the benefit of team members, patients, and families, consistent with agency policy and procedure.
  • Identify the need for staff education and assist in the development of in-service programs for interdisciplinary teams.
  • Assist in planning, delivery, and evaluation of volunteers, students, bereavement, and educational/training programs.

Requirements:

  • Proven track record as a previous Hospice Administrator.
  • Preferred experience of three years in or in a community health service or related field.
  • Demonstrated ability to manage all aspects of terminal care, with specific expertise in crisis intervention, family counseling, pain and symptom control, and bedside nursing.
  • Demonstrated skill in administrative performance, including excellent verbal/written communication skills, knowledge of documentation requirements, and service coordination.
  • Demonstrated ability to execute a supportive and sensitive approach to staff and patient/family needs.
