Job Summary

We are seeking an Independent Optometrist to lease space inside our Walmart stores. As an independent lease holder, you will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule. You will also have the opportunity to opt into equipment, instrumentation, administrative, and/or opt-tech support services for a fair market value.

Key Responsibilities
  • Provide eye and vision care services to patients
  • Take a leading role in patient care with respect to general health and well-being
  • Establish and manage your own practice within the Walmart store
  • Set your own schedule and fees
  • Have the opportunity to opt into support services for a fair market value
Requirements
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
