About This Role

Fresenius Medical Care is seeking a highly skilled Registered Nurse to join our team as a Home Therapies Registered Nurse. As a member of our nephrology healthcare team, you will play a critical role in providing education and support to patients and caregivers who have chosen to dialyze in the comfort of their own homes.

Key Responsibilities
  • Identify and evaluate candidates for the Home Therapies Program, assessing their home environment and readiness for dialysis treatment.
  • Perform assessments and identify barriers to home dialysis, working with patients and caregivers to develop a personalized treatment plan.
  • Coordinate patient care, including assessment, planning, intervention, and evaluation, for an assigned group of hemodialysis patients.
  • Provide initial and ongoing education to patients and families on home dialysis treatment and equipment operation and maintenance.
  • Ensure patient awareness of transplant and treatment modality options.
  • Administer medications as prescribed or in accordance with approved algorithms, documenting medical justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Complete routine patient evaluations within the clinic setting.
  • Ensure correct laboratory collection, processing, and shipping procedures.
  • Collaborate with the Interdisciplinary Team on patient care plans.
  • Make referrals to Social Worker and Registered Dietitian as appropriate.
Requirements
  • Graduate of an accredited School of Nursing.
  • Current appropriate state licensure.
  • Current or successful completion of CPR BLS Certification.
  • Must meet the practice requirements in the state in which employed.
Preferred Qualifications
  • Experience as a Registered Nurse, preferably in chronic or home hemodialysis.
  • Successfully pass the Ishihara Color Blind Test.
Work Environment

The position provides direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. The employee may occasionally be required to move, with assistance, machines and equipment of up to 200 lbs., and may lift chemical and water solutions of up to 30 lbs. as high as 5 feet. The work environment is characteristic of a healthcare facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials. Day-to-day work includes desk work, computer work, interaction with patients, facility/hospital staff, and physicians. The position may require travel to home dialysis patient homes, training sites, or other facilities.
