Patient Care Advocate

We are seeking a compassionate and skilled Registered Nurse/Hospice to join our team as a Care Professional. As a key member of our healthcare staff, you will be responsible for providing exceptional care to geriatric patients in their homes or facilities.
About the Role:
- Deliver personalized care to patients based on their unique needs and preferences
- Collaborate with our Director of Nursing to ensure seamless delivery of care
- Build relationships with patients and their families to understand their individual needs
- Work independently to manage your schedule and case load
Requirements:
- Active RN License in the state of practice
- Valid Driver License and Auto Insurance
- AZ Level One Fingerprint Card (for Arizona applicants)
- COVID Vaccine (with exemption option)
- Flu Vaccine (seasonal)
- CPR or BLS Certification
- Physical Exam (Nevada applicants only)
- Valid TB Test
Compensation and Benefits:
The estimated annual salary for this role is $70,000 - $90,000, depending on location and experience. You will also receive a comprehensive benefits package, including vision and dental insurance, short-term disability coverage, and access to telehealth services.
