About the Role

We are seeking a skilled Occupational Therapist to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in helping patients manage chronic conditions and recover from illness or injury.

Responsibilities
  • Assess patients' daily living and work-related skills, developing therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patient function.
  • Collaborate with healthcare teams to develop and revise treatment plans, ensuring patient-centered care.
  • Provide therapeutic treatment and instruction to patients, improving strength, coordination, and function.
  • Review and expand retraining programs to align with patient goals and rehabilitation program objectives.
  • Consult with vocational team members to develop vocationally oriented plans, promoting patient independence.
  • Communicate patient status and progress to healthcare teams, patients, and families.
  • Participate in care coordination and discharge planning, ensuring seamless transitions.
  • Design community reintegration activities to support patients' physical and psychological recovery.
  • Recommend adaptive equipment to improve patient environments and participation in rehabilitation.
  • Train staff and family members to carry out retraining programs, promoting patient independence.
Requirements
  • Master's degree from an accredited Occupational Therapy program.
  • Minimum six months of occupational therapy experience, with home health experience preferred.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
