Job Title: Licensed Practical Nurse - LPN

We are seeking a dedicated and compassionate Licensed Practical Nurse - LPN to join our team at Consulate Health Care of Norfolk.

About the Role:

As an LPN, you will play a crucial role in providing high-quality nursing care to our residents. You will have the opportunity to collaborate with our healthcare team to implement individualized treatment plans for each assigned resident, ensuring their unique needs are satisfied.

Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and documenting resident information.
  • Supervise and evaluate Certified Nursing Assistants (CNAs) to ensure they are delivering care following our high standards.
  • Oversee fall-risk and pressure ulcer prevention programs, actively working to prevent and manage these conditions.
  • Conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary.
Requirements:
  • Active license to practice as an LPN.
  • Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred.
