Nurse Practitioner

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Hillside, New Jersey.

As a Registered Nurse in our Home Health program, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, ensuring seamless transitions and optimal outcomes.

• Assess and evaluate patients' physical and emotional needs, developing and implementing personalized care plans to address their unique needs.
• Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
• Collaborate with healthcare providers, family members, and caregivers to ensure comprehensive care and support.
• Monitor patients' progress, adjusting care plans as needed to ensure optimal outcomes.
• Document all patient interactions, care plans, and outcomes in our electronic medical record system.

• Current RN licensure in New Jersey.
• 1+ year of experience in home health or a related field.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
