Role Summary:

We are seeking a skilled and compassionate Healthcare Access Coordinator to join our team at Sea Mar Community Health Centers. As a key member of our Managed Care Department, you will play a vital role in ensuring that patients have access to quality healthcare services.

Your primary responsibilities will include:

  • Serving as a liaison between clients and clinic, focusing on alleviating barriers to service and helping patients establish Sea Mar as their medical home.
  • Assisting with public health coverage program applications, ensuring patients have selected a primary care provider, assisting in scheduling appointments and identifying other health care services available.
  • Participating in outreach activities and contributing to corporate marketing campaigns, distributing informational/promotional materials approved by the Managed Care Department.

Requirements:

  • A high school diploma or equivalent is required; an associate's or bachelor's degree in social services is preferred.
  • At least one year of customer service experience is necessary; experience in a healthcare environment is highly valued.
  • Bilingual skills in English/Spanish are highly desirable.
