Nurse Specialist

BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse (RN) to join our team to provide high-quality home health care services to our adult and geriatric clients in the Hazleton area.

• Provide skilled nursing care to clients in their homes, focusing on their unique needs and preferences.
• Develop and implement individualized care plans in collaboration with clients, families, and healthcare teams.
• Conduct thorough assessments, identify potential health risks, and develop strategies to mitigate them.
• Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
• Monitor clients' conditions, report changes to clinical managers, and adjust care plans as needed.
• Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
• Document all interactions, interventions, and evaluations in a timely and accurate manner.

• A current license as a Registered Nurse in Pennsylvania.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience is strongly preferred but not required.
• Graduation from an accredited and approved nursing program.
• Ability to work independently, manage time effectively, and prioritize tasks.
• Strong interpersonal and communication skills.
• Solid computer skills and experience with electronic medical records (EMR) are preferred.
