Palliative Care Specialist

We are seeking a compassionate and empathetic Palliative Care Nurse Practitioner (NP) to join our team in Portsmouth, NH. This role involves providing care to patients in their homes and various facilities in the area.
Key Responsibilities- Conduct comprehensive assessments and coordinate patient care plans with interdisciplinary teams.
- Observe, evaluate, and report on patients' physical and emotional status to nursing supervisors and attending physicians.
- Provide skilled nursing care, including medication administration and treatments as prescribed by physician's plans of care.
- Participate in weekly Interdisciplinary Team Meetings to discuss patients' physical, emotional, psychosocial, and spiritual statuses.
- Maintain and process documentation in compliance with agency standards.
- Utilize computer-based documentation systems and submit required clinical documentation, time records, and other information within specified timeframes.
- Serve as a liaison with institutions to promote continuity of care as patients transition between levels of care.
- Represent the agency at community events and activities.
- A Master's degree, post-masters certificate, or doctoral degree in nursing from a university accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC).
- A current license to practice nursing in New Hampshire and/or Maine where the hospice program operates.
- Certification as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP) from the American Nurses Credentialing Center (ANCC) or the American Association of Nurse Practitioners (AANP).
- Prior experience in hospice or palliative care, clinical oncology, geriatrics, intensive care, or community and family health is preferred.
- Palliative care certification (NBCHPN) or hospice certification (ACHPN) is also preferred.
- Full participation in initial credentialing and subsequent re-credentialing every two years is required.
