Key Responsibilities
The Physical Therapist (PT, DPT, RPT) in Home Health is tasked with assessing and evaluating patient care requirements concerning mobility, endurance, pain, and other health conditions. Utilizing this assessment, the Physical Therapist collaborates to formulate a treatment strategy, implements interventions aimed at improving the patient's overall health, and monitors their progress.
- Conducts evaluations of the patient's physical therapy needs and functional capabilities. Collaborates with the physician to develop a comprehensive therapy plan.
- Monitors and communicates the patient's response to treatment and any changes in their condition to the supervising nurse and/or physician.
- Administers treatments to alleviate pain, restore function, and maintain optimal functioning in accordance with established standards of practice.
- Educates and informs the healthcare team, the patient, and their family/caregivers about the care plan, functional limitations, and progress towards rehabilitation goals.
Required Qualifications
Licensure and Certifications
- Valid Physical Therapy license in the respective state
- Current CPR certification
- Possession of a valid driver's license and vehicle insurance, along with access to reliable transportation
