Occupational Therapist

CenterWell Home Health is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will play a vital role in helping patients manage chronic conditions and recover from injuries or illnesses.

• Confer with patients' physicians and other healthcare team members to develop and revise treatment plans.
• Review and expand retraining programs to meet patients' behavioral goals and rehabilitation program guidelines.
• Report patient progress to physicians, rehabilitation staff, and family members.
• Participate in care coordination and discharge planning activities.
• Design community reintegration activities to assist patients in physical reconditioning and psychological adjustment.
• Recommend and design special adaptive equipment to improve patients' living environments or participation in rehabilitation programs.
• Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills.
• Coordinate and supervise Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
• Accurately document patients' care observations, interventions, and evaluations.

• Degree from an accredited Occupational Therapy Program.
• A minimum of six months of occupational therapy experience preferred.
• Home Health experience a plus.
