Job Summary:We are looking for a highly motivated and experienced Wound Care Coordinator to support our wound care management program. The successful candidate will work closely with our wound care consultant and interdisciplinary team to ensure high-quality wound care services for our patients.
Key Responsibilities:- Conduct weekly wound rounds with wound care consultant and provide updates to medical and family teams.
- Complete weekly skin monitoring and document in PCC.
- Review new admissions and readmissions for accurate wound information.
- Review new or worsening wounds and make recommendations for treatment.
- Coordinate wound care with interdisciplinary team, including dietitian, rehab services, and social services.
- Nursing degree from accredited college or university.
- Registered Nurse with current State License.
- Nurse with Wound Care Certification preferred.
- Previous experience in long-term care setting.
- Previous experience with wound care in a healthcare setting preferred.
