LPN Team Leader for Senior Care

Position: Licensed Practical Nurse in a senior living environment
Key Responsibilities:
- Document clinical observations and patient progress
- Monitor and communicate changes in patient health
- Execute nursing tasks under guidance
- Prepare medical equipment for procedures
- Support patients in self-care practices
- Assist in patient recovery efforts
- Consult with Nursing Supervisor regarding care strategies
- Facilitate assistance with all Activities of Daily Living (ADLs) including bathing, feeding, lifting, and transferring
- Lead initiatives to ensure outstanding patient care
Qualifications:
- Graduate from an accredited Practical/Vocational Nursing Program
- Minimum of one year of recent nursing experience or completion of a refresher course
- Active license as a Practical/Vocational Nurse
- Competence in performing nursing procedures as per the Nurse Practice Act
- Proficiency in the English language
- Valid driver's license and reliable transportation
