Job Summary:We are looking for a dedicated and experienced Registered Nurse to work in our South 5 Telemetry department. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. As a Community Health RN, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with the healthcare team to ensure the highest level of patient care.
Responsibilities:- Assess patient needs and develop individualized care plans
- Provide high-quality patient care and education
- Collaborate with the healthcare team to ensure effective patient care
- Participate in quality improvement initiatives
- Maintain accurate and up-to-date patient records
- Graduate of an accredited School of Nursing
- Current and unrestricted license in good standing to practice as a Registered Nurse in the State of Connecticut or the ability to obtain
- Possess the knowledge and skills necessary to assess health needs of patients and to perform nursing care in a safe, caring manner
- One year of acute care nursing experience
- BSN or ADN/ASN and actively enrolled in a BSN program
- Epic experience
