Job Summary

The Occupational Therapist contracted through Elite Home Health & Hospice is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.

Key Responsibilities
  • Assumes responsibility for ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients, representatives, and caregivers.
  • Provides services ordered by physicians as indicated in plans of care.
  • Provides patient, caregiver, and family counseling.
  • Provides patient and caregiver education.
  • Communicates with physicians responsible for home health plans of care and other healthcare practitioners related to current plans of care.
  • Evaluates patient functional status and home environment for hazards or barriers to independent living.
  • Identifies equipment needs and participates in development of total plans of care.
  • Develops treatment programs and establishes goals for improved function.
  • Teaches new skills or re-trains patients in daily activities lost due to illness or injury, in accordance with organization policy.
  • Maintains appropriate clinical records, clinical notes, and reports changes in patient condition to physicians.
  • Submits records in accordance with organization policy.
  • Participates in HHA's quality assessment and performance improvement program and HHA-sponsored in-service training, case conferences, and other meetings as required by organization policy.
  • Identifies patient and family/caregiver needs for other home health services and refers as appropriate.
  • Instructs patient's family/caregiver and other organization healthcare personnel in patient treatment regimes when indicated and appropriate.
  • Supervises Certified Occupational Therapy Assistants according to organization policy and state regulations.
  • Participates in discharge planning for patients.
Requirements
  • Must be licensed where HHA is located if licensure is required.
  • Must have graduated from an OT program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association (AOTA) or any successor organization of the ACOTE.
  • Must be eligible for or have taken the OT certification examination administered by the National Board for Certification in Occupational Therapy (NBCOT).
  • Two years of clinical experience as an occupational therapist.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR Certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
