Job Summary

The Mental Health Professional II - PRN is responsible for delivering exceptional patient-focused care by providing supervised mental health services to patients and their loved ones. This role involves conducting assessments, offering psycho-education, leading process groups, developing treatment plans, and facilitating therapeutic counseling sessions using evidence-based interventions in one-on-one, family, and group therapy.

Key Responsibilities
  • Conduct one-on-one, group, and family psychoeducational and process groups
  • Develop treatment plans and attend treatment planning meetings and reviews on an ongoing basis
  • Assess environment and patient's safety status and report/respond to actual and potential safety issues or changes in the patient's condition
  • Identify risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)
  • Complete assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope
  • Collaborate with the multidisciplinary team to assess and coordinate patients' discharge planning needs and readiness for discharge
  • Conduct post-discharge follow-up as indicated
Requirements
  • EDUCATION REQUIRED AND/OR PREFERRED: MSW or Master's degree in Counseling/Psychology required
  • EXPERIENCE REQUIRED OR PREFERRED: One year experience in discharge planning in a health care setting preferred
  • SKILLS, KNOWLEDGE, AND ABILITIES: Proven ability to work with others, at all levels within the organization and collaborate effectively. Possess interpersonal problem solving skills and written/oral communication skills. Computer literacy. Ability to complete assessments within the behavioral health field such as but not limited to: psychosocial assessments, discharge assessments and intake assessments
  • LICENSURE/CERTIFICATION: LCSW or LPCA preferred. Current BLS (Basic Life Support) required within 30 days of hire. Current CPI (Crisis Prevention Intervention) required within 45 days of hire. AHCA Level 2 background screen and fingerprinting required
