Optum Home & Community Care is revolutionizing healthcare by integrating industry-leading solutions to create a holistic care model. As a HouseCalls Statewide Traveler, you'll play a vital role in providing annual clinical assessments to patients in the comfort of their homes.
Key Responsibilities:- Conduct in-home assessments, including medical history reviews, system evaluations, medication reconciliations, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing, as appropriate.
- Identify diagnoses to inform care management and active medical management.
- Communicate findings to inform members' primary care physicians of potential care gaps.
- Recognize urgent and emergent situations and intervene accordingly.
- Educate members on disease processes, medications, and compliance.
- Active NP or PA license or ability to obtain by start date.
- Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
- Active national NP or PA certification or ability to obtain national certification in the state of assignment.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- Access to reliable transportation for up to 85% travel within licensed states.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
- Home Health care or home visit experience.
