Job Description

We are seeking a skilled Pediatric Home Health Nurse to join our team at Connect Pediatrics. As a Pediatric Home Health Nurse, you will provide high-quality, patient-centered care to children in the comfort of their own homes. Our team is dedicated to delivering exceptional care and making a positive impact on the lives of our patients and their families.

Key Responsibilities:

  • Provide skilled nursing care to pediatric patients in their homes, following physician-prescribed plans of care.
  • Administer medications, feedings, and treatments as ordered by the physician.
  • Maintain accurate and up-to-date records of patient care, including real-time charting.
  • Collaborate with clinical leadership to coordinate patient care and ensure seamless transitions.

Requirements:

  • Current nursing license (LVN or RN) to practice in the state of Texas.
  • Current CPR Certification.
  • Pediatric experience is preferred but not required.
