We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at the Lebanon Center in Lebanon, NH. As an LPN, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our healthcare team.

About the Role:

As a Licensed Practical Nurse, you will be responsible for providing direct patient care, administering medications, and performing treatments as ordered by physicians. You will also be involved in nursing assessments, care planning, and supervising unlicensed staff.

Responsibilities:
  • Evaluate patients/residents to collect data, observe condition, and report changes in condition.
  • Contribute to nursing assessments and care planning.
  • Administer medications and perform treatments per physician orders.
  • Supervise and monitor patient care provided by unlicensed staff.
