About the Job

Providence is seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team in Seattle, Washington. As an LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Responsibilities

  • Assist the Registered Nurse in the planning and delivery of nursing care
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Provide education and support to patients and their families to promote self-care and independence
  • Collaborate with the healthcare team to ensure seamless care coordination

Requirements

  • Graduate of an accredited Licensed Practical/Vocational Nurse program
  • Washington Licensed Practical Nurse upon hire
  • National Provider BLS - American Heart Association upon hire
  • Ability to work independently and as part of a team
