  • Attend and actively participate in weekly team meetings to discuss patient care and treatment plans.
  • Provide rehabilitation and coordinate medical needs for clients, ensuring their physical and emotional well-being.
  • Monitor and administer medications, including oral medications and needle injections, as prescribed by physicians.
  • Order, review, and monitor medications, ensuring compliance with physician orders and client needs.
  • Assist in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures.
  • Conduct assessments and approve plans of care and service delivery methods, ensuring client-centered care.
  • Perform competency and skills evaluations on employees who provide direct care, ensuring high-quality care.
  • Inform physicians and staff of changes in client medical or mental condition and needs, ensuring timely interventions.
  • Write progress notes, summaries, and discharges, documenting client care and treatment.
  • Implement treatment and rehabilitative activities and strategies, promoting client independence and well-being.
  • Report significant incidents or accidents and notify proper authorities, ensuring client safety.
  • Follow consumer health plans and promote client choice and independence.
Qualifications:
  • Must be 21 years of age.
  • Holds a bachelor's degree in nursing or a related health and human services field from an accredited college or university.
  • Valid driver's license and insurance required.
