Job Summary:The Registered Nurse will be responsible for organizing and directing hospice care, providing comprehensive assessments, and developing individualized care plans. You will work closely with the interdisciplinary team to ensure seamless care coordination and provide emotional support to patients and their families.
Key Responsibilities:- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans
- Provide high-quality patient care, including administering medications, treatments, and performing various nursing procedures
- Collaborate with the interdisciplinary team to ensure seamless care coordination and provide emotional support to patients and their families
- Document patient information accurately and thoroughly, including progress notes, medication administration records, and other relevant data
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Graduate of an accredited school of nursing with a current license in the state
- One to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care
- CPR certification and ability to lift, position, and transfer patients
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist
