The Home Health Registered Nurse plays a crucial role in delivering and documenting personalized nursing care tailored to the physician-directed individualized patient care plan.

This essential care is primarily administered in the patient's residence or a similar environment, but may also extend to external appointments and activities where the nurse accompanies the patient.


Key Responsibilities:
  • Conducts an initial evaluation of the patient and family to assess home care requirements when necessary.
  • Performs comprehensive physical assessments and gathers medical history related to current and past health issues.
  • Regularly reassesses the nursing needs of the patient.
  • Initiates and modifies the care plan as directed, adapting to changes in the patient's condition.
  • Utilizes health assessment data to formulate nursing diagnoses.
  • Sets achievable goals based on nursing diagnoses and integrates therapeutic, preventive, and rehabilitative nursing actions.
  • Involves the patient and family in the development and implementation of the treatment plan and objectives.
  • Implements appropriate preventive and rehabilitative nursing interventions.
  • Administers prescribed medications and treatments as ordered by the physician.
  • Provides healthcare education and instructions to the patient based on assessment and care plan.
  • Identifies discharge planning needs when relevant.
  • Follows home health agency policies and protocols.
  • Participates in case conferences, team meetings, and educational in-services as required.

Minimum Qualifications:
  • Graduate from an accredited nursing program.
  • Current licensure as a Registered Nurse in the applicable state.
  • Adheres to all relevant professional practice standards.
  • Completes Amergis' Competency program as required.
  • Holds current CPR certification if applicable.
  • Submits TB questionnaire, PPD, or chest x-ray as needed.
  • Maintains a current health certificate in accordance with contract or state regulations.
  • Meets all federal, state, and local requirements.
  • Successfully completes new hire training as relevant to the job site.
  • Understands patient confidentiality and HIPAA regulations.
  • Demonstrates effective communication skills to relay information to supervisors, colleagues, and clients; proficiency in English is essential.
  • Possesses computer proficiency.
  • Must be at least 18 years of age.
