Key Responsibilities:
  • Develop and implement individualized plans of care that address patient needs and goals.
  • Collaborate with patients, families, physicians, and other healthcare professionals to coordinate care and services.
  • Provide physical therapy services to patients in their place of residence, including assessment, treatment, and education.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • One year of experience in acute care, rehabilitation, or home care as a Physical Therapist.
  • Registered with the Board of Registration in Massachusetts.
  • Strong customer service skills and ability to work with diverse populations.
  • Excellent verbal, written, and electronic communication skills.
  • Computer proficiency and ability to learn new software.
