Public Health Nurse

Job Summary:
We are seeking a skilled Public Health Nurse to join our team at Elkhart County Human Resources. As a Public Health Nurse, you will be responsible for providing community nursing services in public health clinics, schools, community centers, and homes. Your main areas of responsibility will include immunizations clinics for adults and children, school liaison, and lead case management.
Responsibilities:
- Perform community nursing services in public health clinics, schools, community centers, and homes.
- Provide immunizations clinics for adults and children.
- Act as a school liaison and provide health education to students and staff.
- Lead case management and provide support to patients with chronic conditions.
Requirements:
- Associates degree with four years of experience or BSN required.
- Current Indiana RN License required.
- CPR Certification.
- Valid drivers license and reliable transportation.
- Prior experience in public health a plus.
- Good communication skills required.
- Bilingual (English/Spanish) skills strongly preferred.
- Pre-employment background check required.
