Job Summary

We are seeking a skilled Occupational Therapist to join our team at Abode Hospice and Home Health in Pueblo, CO. As an Occupational Therapist, you will play a vital role in providing high-quality patient care and improving the lives of our patients.

Responsibilities
  • Evaluate patients and develop comprehensive occupational therapy treatment plans in collaboration with physicians.
  • Provide direct and indirect patient care services, including administering therapeutic exercises and instructing patients and caregivers on activities of daily living.
  • Supervise and direct the efforts of Occupational Therapy Assistants as assigned.
  • Conduct comprehensive medication reviews and educate patients and caregivers on medication regimens.
  • Provide patient and caregiver education on safety measures, infection control, and other relevant topics.
  • Collaborate with other disciplines to ensure seamless care coordination and enhance patient outcomes.
Requirements
  • Licensed Occupational Therapist with a strong background in home health care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Proficiency in OASIS documentation.
