Job Summary

We are seeking a dedicated and compassionate Nurse Navigator to join our team at UCHealth, a nationally recognized subspecialty clinic specializing in cerebrospinal fluid (CSF) leaks.

This role is crucial in coordinating care for patients with complex medical histories, requiring a candidate who is passionate about providing comprehensive support throughout their treatment journey.

Key Responsibilities
  • Manage patient referrals and coordinate multidisciplinary care to ensure a seamless experience for our patients.
  • Assist with pre- and post-procedural needs, providing emotional support, counseling, and clinical education to patients and families.
  • Serve as a liaison between patients, clinical staff, administration, physicians, and managed care companies to coordinate resources and resolve barriers to care.
  • Advise patients and families on options and refer to appropriate resources to resolve financial, psycho-social, functional, and administrative issues.
Requirements
  • Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program.
  • Minimum 2 years of nursing experience.
  • Preferred: Bachelor's degree in Nursing.
