Job SummaryThe Registered Nurse coordinates and delivers patient-centered care by assessing patient needs, developing individualized care plans, and collaborating with medical providers to ensure the best possible outcomes. Responsibilities- Assess patient condition during admission and during each shift as scheduled and/or as needed, identifying and reporting any changes in patient status.
- Engage in evidence-based best practices that enhance the patient experience, including SAFETY rounding, communication fundamentals/AIDET, Teach-Back, Narrating Care, etc.
- Ensure patients, physicians, and colleagues have a clear understanding of all communications, and escalate care concerns appropriately.
