About the Role

We are seeking a skilled Occupational Therapist to join our Home Health team. As a Home Health Occupational Therapist, you will play a vital role in helping patients recover from illness, injury, or surgery and regain their independence.

Key Responsibilities

  • Conduct thorough assessments of patients' daily living and work-related skills to develop personalized rehabilitation plans.
  • Administer and interpret diagnostic tests to evaluate patients' level of function and inform treatment decisions.
  • Collaborate with patients' physicians and other healthcare team members to develop and implement comprehensive care plans.
  • Provide individualized therapeutic treatment and instruction to patients to improve strength, coordination, range-of-motion, and function.
  • Review and expand rehabilitation plans to ensure they align with patients' behavioral goals and the overall rehabilitation program.
  • Consult with other healthcare professionals to develop vocationally oriented plans that meet patients' needs and capabilities.
  • Communicate patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and family members.
  • Participate in care coordination and discharge planning activities to ensure seamless transitions and optimal patient outcomes.
  • Design community reintegration activities to support patients' physical reconditioning and psychological adjustment.
  • Recommend and design adaptive equipment to improve patients' living and working environments.
  • Train staff and family members to carry out rehabilitation plans, including dressing, feeding, grooming, and hygiene skills.
  • Supervise, instruct, and evaluate Occupational Therapy Assistants and Home Health Aides to ensure high-quality care.
  • Accurately document patients' care observations, interventions, and evaluations to ensure continuity of care.

Requirements

  • Master's degree from an accredited Occupational Therapy Program.
  • Minimum of six months of occupational therapy experience, with Home Health experience preferred.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
