Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote our clients' health, safety, and independence.

Responsibilities:
  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements, such as mobility, range of motion, and balance assessments.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
  • Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary to promote their health and safety.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Qualifications:
  • A current PA Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
