About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.

Key Responsibilities
  • Evaluate and treat patients with speech, language, hearing, and swallowing disorders
  • Develop and implement individualized treatment plans to meet patient needs
  • Collaborate with healthcare teams to ensure comprehensive care
  • Provide education and training to patients and families on alternative communication systems and strategies
  • Document patient progress and maintain accurate records
Requirements
  • Master's degree in Speech-Language Pathology or related field
  • Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A)
  • Minimum 6 months of experience as a Speech Therapist/Speech-Language Pathologist
  • Home Health experience preferred
  • Current CPR certification
  • Good organizational and communication skills
