Home Health RN Case Manager Job Description

At Interim HealthCare Home Health and Hospice, we are seeking a dedicated and compassionate Registered Nurse to join our team as a Home Health RN Case Manager. As a Home Health RN Case Manager, you will provide skilled nursing care to patients in the comfort of their own homes, promoting independence and improving quality of life.

Responsibilities:
  • Deliver comprehensive patient assessments, wound care, and medication administration in the home setting.
  • Provide patient education to patients and families on various diagnoses, ensuring effective management of conditions at home.
  • Facilitate communication among patient care team members, including family members, therapists, social workers, home health aides, and physicians.
  • Complete accurate and timely documentation of services provided.

Requirements:
  • Registered Nurse with active, unrestricted license in the State of Michigan.
  • At least one year of clinical experience as a Registered Nurse.
  • Current CPR Certification.
  • Reliable transportation with a valid state driver's license and current car insurance.
