Key Responsibilities:- Assess patient needs and develop individualized care plans
- Implement and evaluate patient care plans, ensuring effective communication with interdisciplinary teams
- Provide education and support to patients and families, promoting health and wellness
- Collaborate with healthcare teams to ensure seamless care transitions
- Participate in quality improvement initiatives, promoting best practices and patient safety
- Current State of PA RN licensure or multi-state licensure through the enhanced Nurse Licensure Compact (eNLC)
- CPR certification
- Act 34, Act 33, and Act 73 clearances
- Certification in area of specialty (preferred)
