POSITION OVERVIEW
Under the supervision of the Director of Practice Management, Practice Managers, and Providers, the Licensed Practical Nurse (LPN) will play a vital role in patient evaluation and care. This includes maintaining accurate documentation, ensuring the clinical environment is well-organized, and executing various clinical tasks.
The LPN is committed to delivering exceptional service that enhances the experience of patients and their families, demonstrating a thorough understanding of patient-centered care.
REPORTING STRUCTURE
Reports To:
Director of Practice Management, Practice Managers, and Providers.
Supervisory Responsibilities:
None
Collaborative Relationships:
Work closely with MMA Administration, Providers, office personnel, and patients.
KEY RESPONSIBILITIES
- Conduct nursing assessments relevant to the patient's primary complaint, including vital signs and other necessary measurements, documenting findings in the medical record and notifying the physician of any significant issues.
- Review and document the patient's medication history during each visit.
- Prepare patients physically and emotionally for examinations and procedures, providing appropriate explanations and reassurances based on the patient's age.
- Deliver patient education tailored to identified needs, covering topics such as diagnoses, procedures, and home care.
- Operate within the scope of education and practice policies.
- Order and maintain necessary clinical supplies, monitoring inventory to prevent loss.
- Ensure that all equipment is properly maintained and cleaned according to established guidelines.
- Exhibit knowledge of sterile techniques and universal precautions as required.
- Perform laboratory tests as ordered by the physician and ensure follow-up documentation.
- Welcome patients and escort them to the examination area.
- Conduct the history and physical examination of patients, documenting findings appropriately.
- Assist physicians during procedures as directed.
- Prepare and sanitize the patient area as needed, including setting up equipment and cleaning post-procedure.
- Administer and document medications as prescribed by the physician.
- Assist with patient discharge processes.
- Conduct telephone triage, obtaining orders from physicians when necessary and communicating results back to patients.
- Process prescription requests to pharmacies, ensuring compliance with regulations regarding controlled substances.
- Maintain an adequate supply of medications for daily operations.
- Ensure that consent forms are completed for necessary procedures.
- Exhibit professionalism in all communications with patients, physicians, and staff.
- Log sample medications in accordance with policy.
- Establish and maintain a medical records system that complies with established procedures.
Maintain confidentiality of patient information and uphold patient privacy rights.
- Contribute to the development of policies and procedures.
- Assist in creating a clean and safe environment for patients and colleagues.
- Identify clinical concerns and recommend solutions or take necessary actions.
- Demonstrate adaptability in response to changes in workload and staffing.
- Perform additional duties as assigned by the Practice Manager or Physician.
Uphold patient safety standards to ensure a secure environment and effective use of equipment and technology.
- Support the mission, vision, and values of Memorial Healthcare, adhering to all policies and ethical standards.

QUALIFICATIONS
EDUCATION
- Graduate of an accredited nursing program with a current LPN license.
EXPERIENCE
- Minimum of one (1) year of experience in a general medicine/surgical setting, interacting with diverse patient populations.
These requirements are not exhaustive and may be supplemented as needed.
Sedentary Work:
Regularly required to stand, kneel, and crouch. Daily activities may include moving about, sitting, climbing stairs, and bending. Must be able to lift and carry up to 20 lbs.
Vision:
Ability to perceive objects clearly.
Motor Coordination:
Regularly perform tasks requiring hand and finger dexterity, including using tools and equipment. Must be able to navigate the hospital independently.
