Job Summary

The Social Worker – Inpatient Behavioral Health is a key member of our Behavioral Health team, responsible for providing clinical social work services and case management support to patients on our West 5 unit and their families.

Key Responsibilities
  • Assess and address clinical treatment goals for patients, ensuring continuity of care through collaboration with community providers and hospital programs.
  • Provide family education, bio-psychosocial assessments, and group therapy sessions as part of treatment planning and implementation.
  • Develop and maintain relationships with community providers and other hospital programs to ensure seamless care transitions.
  • Contribute to discharge planning activities, ensuring patients receive necessary support and resources upon leaving the hospital.
Requirements

This role requires a strong understanding of clinical social work principles and practices, as well as excellent communication and interpersonal skills. The ideal candidate will be able to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.

This position reports to the Assistant Clinical Manager, Behavioral Health and operates within established organizational and departmental policies and procedures.
