We are seeking a highly skilled and compassionate Care Manager to lead our team of caregivers in providing exceptional client care services. As a key member of our team, you will be responsible for guiding our clients towards their best state of wellness, while fostering a culture of professionalism, integrity, and compassion.
Key Responsibilities- Holistic Assessment and Planning: Oversee the comprehensive assessment and evaluation of clients' needs, working closely with families to achieve and maintain optimal wellness.
- Resource Facilitation: Ensure clients receive the right resources and support for their care and wellbeing by facilitating interactions with various providers.
- Strategic Decision-Making: Take charge of decisions related to resource allocation, approach, and tactical operations within your department, driving forward key projects and initiatives.
- Collaboration and Teamwork: Engage in cross-departmental collaboration, participating in meetings with fellow team members to align our efforts with company objectives.
- Regional Connectivity: Maintain regular contact with other care managers across the region, fostering a network of knowledge and support.
- Team Stewardship: Lead the recruitment, onboarding, and, when necessary, the termination of caregivers within your scope, ensuring our team is built on excellence.
- Relevant Experience: Ideally, you'll have 2+ years of experience in home care, showcasing your dedication to exceptional client service.
- Educational Foundation: A high school diploma is required, while a bachelor's degree or equivalent work experience is preferred.
- Administrative Acumen: Demonstrate office and administrative support skills to manage the intricacies of your role.
- Effective Communication: Your excellent written and verbal communication skills are crucial in guiding your team and interacting with clients and families.
- Mobility: A reliable vehicle is essential for travel within your designated area.
