At CenterWell Home Health, we prioritize health and well-being. As a Home Health Physical Therapist, your role will encompass:

Responsibilities:

  • Designing and executing tailored physical therapy interventions for patients facing various health challenges, aiming to restore optimal physical function.
  • Conducting assessments to evaluate patients' physical capabilities, aiding physicians in determining functional levels and documenting findings to inform treatment plans.
  • Creating and implementing rehabilitation programs aligned with the physician's treatment plan and the overall objectives of the patient care team.
  • Collaborating with physicians and clinical colleagues to gather comprehensive patient information, enhancing the development and adjustment of therapy strategies.
  • Providing clear, written instructions and care plans to Physical Therapy Assistants and Home Health Aides, ensuring adherence to the current therapy objectives while supervising their performance.
  • Monitoring the documentation process by therapy assistants and aides, fulfilling supervisory responsibilities effectively.
  • Ensuring accurate and timely documentation of patient care activities, including interventions and evaluations, while facilitating the prompt processing of physician orders.
  • Communicating patient progress to physicians, clinical managers, and family members, while submitting necessary reports and summaries to the care management team.

Qualifications:

  • Degree from an accredited Physical Therapy Program recognized by the APTA.
  • Preferred experience of at least one year in physical therapy.
  • Possession of a current and unrestricted Physical Therapy license.
