Veteran Healthcare Primary Care Physician

Position Overview: We are seeking a committed Primary Care Physician to serve the healthcare needs of Veterans at the Department of Veteran Affairs. In this role, you will collaborate with a diverse PACT team to deliver both urgent and ongoing medical care to our Veteran population. Your expertise will be crucial in implementing the PACT model and adhering to clinical standards to enhance patient care results and foster high-functioning teams.
Key Responsibilities:
- Provide continuous care for a designated group of Veterans
- Utilize effective patient identification tools
- Ensure timely evaluations and access for patients
- Engage in initiatives aimed at improving team performance
- Deliver health education and support to patients
- Encourage patient involvement in their healthcare decisions
- Maintain clear communication with team members and patients
- Administer clinically necessary services to patients
- Guarantee same-day access for patient appointments
Work Schedule: Monday - Friday (8:00 a.m. - 4:30 p.m.)
