Job Summary:
We are seeking a skilled Nurse Practitioner to join our team at Memorial Health University Medical Center. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and coordinating comprehensive care for acutely ill patients.
Responsibilities:
- Assess patients health status through interview, observation, and physical assessment skills.
- Develop an individualized plan of care with specific goals and target dates in collaboration with the patient, family, provider, and other members of the healthcare team as appropriate.
- Implement appropriate interventions identified in the plan of care to move the patient toward established goals or outcomes.
- Systematically evaluate and document patients progress to expected outcomes/goals on an ongoing basis throughout the shift and modify plan of care based on patients responses to expected outcomes.
- Complete documentation in the patient medical record accurately and in a timely manner.
- Demonstrate technical knowledge and skill to safely utilize patient care equipment.
- Prioritize work based on changes in the patient condition, changes in assignments, and unanticipated interruptions.
- Collaborate with the healthcare team to organize tasks and activities from the patients admission through discharge. Delegate tasks and activities to other members in the healthcare team using appropriate.
- Perform all nursing care activities using techniques to promote safety, reduce risks, and follow policies and procedures.
- Act as an advocate for the patient with families, physicians, and other healthcare team members.
- Adapt teaching strategies based on the patients response, readiness to learn, and level of comprehension.
- Implement cultural, age-specific, gender, and developmental factors into patient/family education about health concerns, disease-specific conditions, self-care, and long-term management of chronic conditions.
- Promptly follow up on patients/family requests/concerns.
- Provide care to all age groups as defined by nursing standards on each unit. Use age-appropriate communication techniques.
- Perform tactics to support great patient experience and lead others to support initiatives in patient experience.
Qualifications:
- Diploma or Associates Degree in Nursing required.
- BSN preferred.
- Licensed in the State of Illinois.
- Certified in CPR or completes certification during Nursing Team Onboarding.
- Certified in ACLS or completed within 6 months of employment.
Other Knowledge/Skills/Abilities:
- Advanced skills in reading, writing, and oral communication.
- Demonstrates knowledge of basic skills of professional nurse.
- Demonstrates skills in the use of clinical information systems, basic skills in keyboarding, and use of computer systems.
