Job Title: Hybrid RN Patient Care Manager

We are seeking a skilled and compassionate Hybrid RN Patient Care Manager to join our team at LHC Group. As a key member of our Home Health team, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Responsibilities:
  • Supervise and coordinate clinical services to ensure the highest level of patient care.
  • Provide and direct provisions of nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
  • Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
  • Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
Qualifications:
  • Current RN licensure in the state of practice.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
