We are seeking a skilled and compassionate Hospice Nurse to join our team at SSM Health. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families in their homes, community-based settings, and other care facilities.
Key Responsibilities- Assess and coordinate patient care needs with the healthcare team
- Perform point-of-care testing and provide skilled nursing care within established plans of care
- Collaborate with patients, families, and healthcare teams to develop comprehensive plans of care
- Document visit notes, treatments, and care in a timely and accurate manner
- Respond to patient and family needs, issues, or concerns in a timely and professional manner
- Graduate of an accredited school of nursing or equivalent education
- Current RN licensure in the state of employment
- Basic Life Support (BLS) certification
- Valid driver's license and reliable transportation
This is a PRN position, working variable hours to meet patient needs. You will be required to travel to patient homes, community-based settings, and other care facilities to provide care and support.
