We are seeking a skilled Physical Therapist to join our team at Bridge Home Health and Hospice. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to our organization's mission to deliver exceptional healthcare services.
Key Responsibilities- Assess and evaluate patients' therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
- Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
- Direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and determine priority needs for physical therapy.
- Report to physicians patients' reactions to treatment or changes in condition, and initiate physical therapy programs.
- Train patients in the use of prosthetic devices, identify patient and family/caregiver needs for other home health services, and refer as necessary.
- Prepare and submit clinical and progress summaries based on the attainment of goals, and participate in discharge planning for patients.
- Possess a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
- Licensed to practice as a physical therapist by the Physical Therapy Board of California.
- Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
- Demonstrate good verbal and written communication and organization skills, and possess and maintain current CPR certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
