Job Summary:
The Community Health Worker will work under the supervision of the Chronic Disease Prevention Coordinator to address the growing burden of chronic diseases and associated risk factors. This position will provide coordination of chronic disease prevention activities utilizing a policy, systems, and environmental change approach and a health equity lens.
Key Responsibilities:
- Provide client management services for persons diagnosed with primary hypertension, including blood pressure monitoring, control support, and education.
- Support community outreach events, including recruitment of patients, screening, and health education and promotion activities.
- Serve as a liaison for the HPI, Live Healthy DeKalb Coalition, and the community.
- Participate in outreach events such as health fairs and community events to promote vaccinations.
- Develop and maintain relationships with internal and external partners.
- Work with community organizations, coalitions, schools, and healthcare systems to address policy and environmental changes.
- Collaborate with partners to determine community health needs and the availability of services.
- Attend required training and other pertinent meetings, organize lectures, and host educational conferences and seminars.
- Maintain knowledge of current trends and developments in the field.
- Maintain databases, mailing lists, telephone networks, and other information to facilitate health education programs.
Requirements:
Must have a strong understanding of chronic disease prevention and health equity principles. Must be able to work effectively in a team environment and communicate effectively with diverse stakeholders. Must have excellent organizational and time management skills.
