Job Title: Addiction Medicine Nurse Practitioner

County of Wood is seeking a skilled Addiction Medicine Nurse Practitioner to join our team. As a key member of our justice involved care team, you will provide medical and educational services to individuals according to State of Wisconsin regulations and within the scope of the Nurse Practitioner License in the State of Wisconsin.

Responsibilities:
  • Obtain addiction medicine focused medical history and physical data to ensure accurate patient records.
  • Perform appropriate physical examinations and record findings in patient records.
  • Interpret data to determine and order appropriate laboratory and diagnostic tests.
  • Establish a diagnosis and treatment plan based on synthesized data.
  • Provide information to individuals and their care team concerning medications, plan of care, and treatment regimes.
  • Work collaboratively with care team members to ensure complete delivery of prescribed care.
  • Arrange referrals to other health care professionals and facilities as necessary.
  • Provide health education to individuals and their care team about ways to improve, promote, and maintain their health status.
  • Maintain accurate records, medication lists, and documentation of care and follow-up for administrative purposes and reimbursement of services.
  • Adhere to the drug formulary used by the practice to be compliant with agreement.
  • Identify barriers to patient care flow or delivery and assist in formulating corrective action.
  • Oversee and intervene with clinical issues and support/direct triage by clinical staff when necessary.
  • Demonstrate responsibility for individual performance and efficient utilization of resources to ensure timely completion of duties and to promote financial viability.
  • Follow established safety precautions and procedures in the performance of all duties in order to ensure a safe environment.
  • Serve as the primary care practitioner to assigned patients and act as a primary care Consultant to other health practitioners.
  • Complete history and physical examinations, perform differential diagnostic assessments, treatment interventions, preventative health screenings, and patient education.
  • Provide recommendations for post-release primary care and participate in care coordination with community-based care providers.
  • Provide necessary information to care team regarding addictive disorders and co-occurring or co-morbidity health concerns.
  • Utilize appropriate medication based on best practices.
Requirements:
  • Two years of Advanced Practice Nurse Practitioner experience.
  • Experience with prescribing medication-assisted treatment medications, specifically buprenorphine and naltrexone-containing products.
  • One year in addiction medicine team-based care specialty, preferred.
  • Basic everyday living skills, including the ability to understand and follow directions, and reading and writing is necessary.
  • Ability to communicate orally and in writing with individuals and groups.
  • Ability to maintain confidentiality.
