Job Overview

This role involves working with a high-performing Inpatient Care Management team to coordinate and manage patient care across the continuum. As a Clinical Care Coordinator, you will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services needed to meet the member's health needs.

Key Responsibilities
  • Care Coordination: Through onsite or electronic concurrent review, assess and develop individualized plans of care that address the unique needs and goals of each patient.
  • Discharge Planning: Ensure timely discharge planning and coordination with various stakeholders to facilitate smooth transitions and optimal outcomes.
  • Clinical Decision Making: Utilize clinical knowledge, problem-solving skills, and decision-making expertise to evaluate requests for authorization, verify eligibility, interpret benefits, and determine medical necessity.
  • Communication: Regularly communicate with internal and external partners to facilitate clinical decisions regarding medical necessity and timely access to care.
Requirements
  • Education: Bachelor's degree in Nursing preferred; current California RN or LVN license required.
  • Experience: Minimum three years' recent acute clinical experience in a hospital setting.
  • Skills: Managed care experience desirable; basic computer skills, including electronic medical records, Outlook email, and Microsoft Office suite.
About Your Work Environment

You will be based in various regions in California, with requirements to visit local hospitals. A valid California driver's license and reliable vehicle are essential for this role.

Compensation

The anticipated pay range for this role is $85,000 - $110,000 per annum, depending on qualifications and experience.
