POSITION AVAILABLE:
Full-Time Wound Care Nurse (Monday to Friday, on-call weekends once a month)

Role Overview
In alignment with our mission to enhance the quality of life for our Residents, the Wound/Treatment Nurse plays a vital role in delivering outstanding nursing care and customer service. This position is responsible for executing skin treatments for all Residents under their supervision. The Wound Nurse will obtain treatment directives from attending physicians and assist in adjusting treatment plans according to established protocols.

Key Responsibilities

DAILY TASKS

Conduct a comprehensive assessment of all new admissions and re-admissions to identify any open wounds. Ensure that treatment sheets, orders, and preventative measures are established.

- Monitor residents with new skin issues and ensure the implementation of treatment plans, including updating care plans as necessary.

WEEKLY TASKS

- Update treatment sheets weekly and ensure compliance by the end of the designated day.

- Unit Managers are responsible for updating care plans weekly for pressure and non-pressure wounds.

- Submit a Wound Report detailing the percentage of Center Acquired Pressure Ulcers.

- Administer treatments for vascular, surgical, and pressure wounds of stage 2 or greater.

- Charge Nurses are tasked with treating stage 1 skin tears and ensuring weekly updates of skin sheets.

ONGOING RESPONSIBILITIES

- The Wound Nurse is responsible for proactive wound prevention through daily staff education and monitoring.

- Adjust treatment plans as necessary in collaboration with the Wound physician and primary care providers.

- Implement preventative measures for residents at risk of developing pressure ulcers, including appropriate assessments and care plans.

1. Collaborate with the nursing team to assess and develop skin care treatment plans.

2. Initiate referrals as needed.

3. Evaluate residents upon admission to identify normal versus abnormal findings.

4. Provide supervision to C.N.A.s and ensure task completion.

5. Administer medications within the scope of practice.

6. Maintain accurate and timely documentation of resident care.

7. Ensure adherence to established care plans and assess resident status regularly.

8. Participate in weekly rounds with the Wound physician.

9. Maintain infection control and safety protocols during care delivery.

10. Foster a caring environment that respects the dignity and privacy of residents.

11. Respond promptly to resident needs and inquiries.

12. Engage in departmental meetings and training sessions as required.

Qualifications

  • Graduation from an accredited nursing program.
  • Current State License as a Registered Nurse or Licensed Practical Nurse.
  • C.P.R. Certification.
